Emergency veterinary authorization

1. Authorization

You authorize Dave and Shelly (DSPetSitting) to obtain veterinary examination, treatment, diagnostics, hospitalization, and surgery for your pet(s) as recommended by a licensed veterinarian while your pet is in our care.

2. Financial responsibility

You agree to reimburse DSPetSitting for any and all veterinary fees and charges — including emergency care — incurred for your pet(s) while under our care. You remain the financially responsible party with the clinic.

3. Preferred and after-hours clinics

Provide your preferred clinic and after-hours ER on your pet intake / account. If those clinics are unavailable or delay would endanger your pet, you authorize care at any reasonably available emergency veterinary facility.

4. Contact attempts

We will attempt to reach you (and your emergency contacts when listed) when practicable before non-emergency procedures. In a true emergency, we may proceed with recommended care if we cannot reach you in time.

5. Euthanasia

Draft default: euthanasia will not be authorized without your prior verbal or written consent, except as a Nebraska attorney or clinic ethics may require in extreme suffering cases. Attorney will finalize this clause.

6. Meet ’n greets

Complimentary meet ’n greets are short courtesy visits. The same financial authorization applies if veterinary care becomes necessary during a visit on our premises.
